Changes to Vancomycin Therapeutic &Critical Values

Effective immediately, the following changes will be adopted by the laboratory with regard to Vancomycin test reporting.
1) Trough ranges, previously reported as 5-10 ug/mL, will now be reported as 10-20 ug/mL. Any value greater than 20 ug/mL will be considered a critical value and will be called by the Laboratory.
2) Peak ranges will continue to be reported as 30-40 ug/mL. Any value greater than 40 ug/mL will be considered a critical value and will be called by the Laboratory.
3) Random test results will continue to be reported without a range. Any value greater than 40 ug/mL will be considered a critical value and will be called.
