New STHL Consent Form Now in Use

Southern Tier HealthLink (STHL) has made an important change in their patient consent form and and UHS sites should be using the new form. To help clarify for patients which organization they are giving consent to when they sign a STHL consent form, forms are now required on an organizational (or “legal entity”) basis.

One form is required for all of UHS (as a legal entity). New STHL UHS consent forms can be identified by the list of UHS legal entities at the top of the form. STHL Consent Forms are currently being collected at many UHS locations, including outpatient registration at WMH, BGH, CMH and DVH, UHS Primary Care and Medical Group practices, as well as other associated clinics. STHL has collected over 40,000 STHL Consent Forms at UHS and other area healthcare organizations.
