The consent for operation form will soon include a space to identify a first assistant physician (when applicable) for the surgery/procedure, as well as provision for the patient to refuse the presence of a sales representative in the operating room. Space will be included for the name of the sales representative involved when the patient is agreeable to his/her presence. There will be provision for the patient to refuse being filmed and/or photographed during the procedure. The new form will be implemented following education for physicians and nursing.
Documentation must be present on all consents of the name of the physician performing the procedure and/or explaining the procedure to the patient. Blank spaces must be completed prior to the procedure. This was a DOH finding during the last survey.
