Southern Tier HealthLink (STHL) is pleased to support two New York Medicaid Health Home projects in our community. Catholic Charities of Broome County and United Health Services will each lead their own community network of health care and behavioral health providers, as well as social service organizations to provide comprehensive services to select Medicaid beneficiaries in Broome County. Both projects are expected to begin accepting patients in January 2013.
STHL will play an important role in both programs by providing health IT services to the lead agencies and their community partners, enabling care coordinators to more readily track patients' progress, communicate with each patient's providers, and follow up with the patient. STHL services include access to STHL's Web Portal, document exchange between STHL and electronic medical record systems, and care management services, including alerts/notifications following ER visits and a secure messaging system.
